2nd Booster shots are currently limited to adults and adolecents only (Minimum age 12). (Minimum distance to your last vaccination or contact to the virus is six month, except for those who received the Johnson&Johnson vaccine. For these patients the minimum distance is 4 weeks)
Instructions for patients with a vaccination appointment
For those patients who have a vaccination appointment from us the following documents for information and consent are available for download below.
Please inform us if you develop a fever (= temperature above 38.5 ° C) the day before the vaccination. If you have been infected with the virus in the waiting period between the invitation and the vaccination, please also inform us. Your vaccination appointment will then be postponed. Please see the documents below for more details.
Documents that you should bring prefilled with you to the vaccination:
If you have an appointment to be vaccinated with Comirnaty (BioNTech) omicron booster:
- Information sheet for mRNA vaccines (BioNtech / Pfizer or Moderna) and informed consent form (just read this document at home)
- Consent form and medical questionaire (for booster appointments -please bring this document filled out in advance to the appointment)
- Vaccination booklet (if not available, this can be purchased at my office)
You will receive a digital vaccination certificate at my office following the vaccine application.
As of October 11th 2022 we will exclusively vaccinate with the Pfizer/Biontech Corminaty Original/BA.4/5 vaccine, which is a mix of the original mRNA-vaccine by Biontech and a an Omicron BA.4/5 variant adapted new component in a 1:1 mix (15µg Original + 15 µg BA.4/5).
